A few drops of urine during a run, a gym class or a game of tennis can change how you move. Many women start avoiding jumping, lifting, coughing in public or even drinking water before exercise. Bladder leakage during exercise in women is common, but it is not something you simply have to put up with.

This symptom is often a sign that the pelvic floor needs assessment and support. It can affect women at any age, including active women who are fit, strong and otherwise well. Non-surgical treatment options are available, with the appropriate approach depending on the cause and pattern of symptoms.

Why does exercise trigger bladder leakage?

Leakage with movement is most commonly known as stress urinary incontinence. The word “stress” refers to physical pressure on the bladder, not emotional stress. When you laugh, cough, lift, run or land from a jump, pressure inside the abdomen rises quickly. If the pelvic floor muscles and supporting tissues cannot respond with enough strength and timing, urine can escape.

The pelvic floor is a group of muscles at the base of the pelvis. These muscles help support the bladder, bowel and uterus, while also helping close the urethra, the tube that carries urine out of the body. They need to be strong, coordinated and able to react automatically during movement.

High-impact exercise is a common trigger because repeated landing creates rapid force through the abdomen and pelvic floor. Leakage can also occur during lower-impact activities, such as brisk walking, Pilates, cycling or strength training, particularly when breath is held during effort.

A small leak may feel insignificant at first. But if it affects the way you exercise, socialise, dress or feel about your body, it deserves proper attention.

Who is more likely to experience bladder leakage during exercise?

Pregnancy and vaginal birth can stretch and strain pelvic floor tissues, particularly after a long labour, assisted delivery or a larger baby. Symptoms may begin soon after birth, but some women notice leakage only years later when they return to running, sport or heavier gym training.

Perimenopause and menopause can also play a role. Changes in oestrogen can affect the tissues around the urethra and vagina, while age-related changes in muscle strength and connective tissue may reduce pelvic support. This does not mean leakage is inevitable with age. It means the underlying causes need to be considered carefully.

Other contributing factors include chronic coughing, constipation and straining, carrying excess weight, previous pelvic surgery, high-impact sport and repeated heavy lifting. Some women have several contributing factors, while others have no obvious cause beyond pelvic floor weakness or poor muscle coordination.

Fitness does not guarantee pelvic floor function. A woman may have excellent cardiovascular fitness and strong legs or abdominal muscles, yet still have a pelvic floor that is underactive, overworked or not responding at the right moment.

Do not assume Kegels are the only answer

Pelvic floor exercises can be helpful when they are prescribed appropriately and performed consistently. However, many women are unsure whether they are contracting the right muscles. Others hold their breath, tighten their buttocks or abdominal muscles instead, or perform exercises that do not match their specific problem.

More effort is not always better. Some women have a pelvic floor that is tense and poorly coordinated rather than simply weak. In that situation, repeatedly squeezing without assessment may not address the problem. The right approach depends on your symptoms, medical history, pelvic floor function and the type of activity that causes leakage.

A clinical assessment can help distinguish stress urinary incontinence from urge incontinence, where a sudden overwhelming need to pass urine leads to leakage. Some women experience both. It is also important to identify symptoms that may need further investigation, including pain, recurrent urinary tract infections, blood in the urine, difficulty emptying the bladder or new neurological symptoms.

Practical changes that may reduce leaks while you seek care

You do not need to stop exercising altogether. Movement supports physical health, mood, bone strength and confidence. Instead, consider modifying the activities that reliably trigger symptoms while you work on treatment.

Try reducing high-impact intervals temporarily, shortening running strides, choosing lower-impact cardio on some days and building intensity gradually. During strength work, breathe out through the effort rather than holding your breath. For some women, learning to gently activate the pelvic floor before a cough, lift or jump can improve control.

Bladder habits also matter. Avoiding fluids all day can irritate the bladder and make urine more concentrated, so aim for regular hydration rather than restricting water before a workout. If caffeine or alcohol worsens urgency, reducing these may help. Addressing constipation is equally worthwhile, as regular straining places ongoing pressure on the pelvic floor.

Pads can provide short-term reassurance, but they should not be the whole plan. Choosing increasingly absorbent products while giving up activities you enjoy can allow a treatable condition to take up more space in your life.
